Then the issue is in the alert category configuration itself, since normally when we want to test out Alert Categories, we run that report after the configuration and we will receive a dummy alert at our receipient.
Can you check SCOT, Alert Category configuration once.
Also, please check whether you have enabled the classical alert mechanism at the IE level like below:
go to tcode sxmb_adm
select Integration Engine Configuration. category is monitor.
set ALERTING_IS_ACTIVE current value to 1.
set ALERTING_TARGET current value to 0 ( to use Alert Framework mechanism)
Please try this out and let me know if you still having issues.
